Please read the following waiver carefully. It includes release of liability and waiver of legal rights. By agreeing electronically, you acknowledge that you have both read and understood all text presented to you as part of the registration process.

REGISTRATION: I hereby confirm that I am the parent or guardian of the enrolled child. By registering for a LogiAcademy class, I give permission to let the enrolled child participate in the specified LogiAcademy Program. All provided information included in the registration is correct to my knowledge, and all contact information provided can be used to communicate with me.

By submitting this registration, I am granting to LogiAcademy permission to send me electronic mail. I understand that I can revoke permission to mail to my email address using the unsubscribe feature, or by replying “unsubscribe” at any time.

CONDUCT: By completing the registration, I agree that my enrolled child and family members will abide by the code of conduct of the school, school district, or other facility where the class is held, and that my enrolled child will abide by the instructions of the LogiAcademy instructor and will not be disruptive during the LogiAcademy Program. I acknowledge and understand that improper conduct of the enrolled child and/or family members may result in disciplinary action as determined by the school, school district or facility where the class is held. I acknowledge and understand that improper conduct during a LogiAcademy Program may result in temporary or permanent removal of the enrolled child from the LogiAcademy Program as determined by the LogiAcademy instructor in his or her sole discretion.

I understand that I am solely responsible for my child’s arrival to and departure from the LogiAcademy program and will comply with all requirements set forth by the school, school district, or other facility to attend and arrive at and depart from class. I will arrange for prompt pick up of my child once the LogiAcademy Program is over. I understand that the LogiAcademy instructor is not responsible for supervising my child at the facility once the LogiAcademy Program is over, or for overseeing where my child goes and/or with whom they leave after dismissal from the LogiAcademy Program.

I understand that in the event my child is unable to attend one or more of the LogiAcademy classes for any reason, I will not be entitled to a refund for these absences. In the event of exceptional circumstances that will prevent my child from attending the balance of the classes, I understand that I can contact LogiAcademy to request a refund, but whether a refund will be given will be in the discretion of LogiAcademy.

RELEASE OF LIABILITY: In consideration for allowing my child to enroll in the LogiAcademy Program, I hereby release and forever discharge and hold harmless LogiAcademy and their respective officers, employees and agents from any and all claims or liabilities arising from or in connection with any accident or injury (including injury caused by negligence) or illness occurring while my child is attending the LogiAcademy Program or at the facility where the program is conducted before, during or after the Program.
